Forms Needed for CPA Licensure
Congratulations on passing the CPA Exam! Below are several forms you will need to apply for your CPA license in New Jersey:
- Statement of Experience
Once you have completed the required one year of experience in public accounting, or its equivalent under the direction of an active licensee and the experience includes evidence of intensive and diversified experience in accounting or auditing, the Statement of Experience form must be completed by your employer.
- Criminal History Information
Complete the Request for Criminal History Record Information form. Be sure to insert your name, current home address including zip code, and social security number.
